If you wish to fill out intake paperwork prior to your appointment, please first review the Client/Therapist Agreement and the Notice of Privacy Practices.  These are your copies to keep.  Please print, complete, and bring the Registration and Acknowledgment Form, Social Media Policy, Tele Therapy Consent, Credit Card Retainment Consent, Release of 

Information and Court Appearance Agreement with you to your appointment.  Please also 

bring your photo ID and insurance card to your appointment; our office will need to obtain a copy of each upon your arrival. Please also be prepared to pay for services prior to your appointment time.
